Equipment Repair Form

Electro-Acuscope and Myopulse Equipment Line Service and Repairs. Below is the procedure for sending Electro-Acuscope, Myopulse equipment and/or accessories in for service.

1. Complete this Service Request Form (PDF).

2. Make and keep a copy of the form for your records.

3. Fax Service Form to (866) 319-1361. Thorp Institute must receive record of your serial number in order to track the your repair and return your machine!!!

4. Place completed form in package with equipment and charger. (Always include charger module for testing. *Note on your repair form that the charger is included.)

5. Pack equipment properly (double boxed, preferably, in original packing materials, otherwise lots of padding is recommended, bubbles are not optimal as handles may break.)

6. Ship Instrument with Service Request Form to Equipment Manufacturer:

Repairs take an average of 4 weeks. You will be invoiced for your repair prior to shipment of repaired equipment. Thorp Institute must receive full payment for repair prior to equipment being returned.
